چکیده

Medial canthal tendon (MCT) laxity is a common condition, usually age related and often causing symptoms of epiphora, discharge, irritation, and redness. MCT repair is more complicated than that of its lateral counterpart because of the intimate relation with the canaliculus (Fig 1). The position and patency of the canaliculus can be affected by any surgery to the canaliculus. Because of the problems with surgical repair, surgery is often delayed until the MCT laxity is advanced (Fig 2) For this clinical controversy, a panel of five eyelid surgeons was invited from Canada, England, France, and the United States to give their management for the symptomatic patient with MCT laxity and their surgical techniques according to the degree of laxity (Table 1).

Posterior Medial Canthal Thermoplasty.

PURPOSE To describe a new, simple, quick, and minimally invasive surgical technique for managing medial canthal tendon laxity. METHODS The technique works by creating a directed posterior lamellar scar. Sutures are preplaced in firm medial canthal tissue, and a triangular diathermy burn is made between this fixation point and the medial tarsal plate (sparing the canaliculus). This area is the...

Upper eyelid rotation flap for reconstruction of medial canthal defects.

M edial canthal defects resulting from cancer resection after Mohs micrographic surgery present challenging reconstruction dilemmas. In fact, medial canthal reconstruction has been controversial because of the time-honored method of second-intention healing in wound management of this area. Yet, small flaps and grafts for repair of cutaneous medial canthal defects may result in a quicker healin...

Management of a type II nasoethmoid orbital fracture and near-penetration of the intracranial cavity with transnasal canthopexy.

Nasoethmoid orbital fractures are perhaps the most complicated aspect of craniomaxillofacial trauma. Involvement of the medial canthal tendon markedly increases the complexity of the repair. We report a case of type II nasoethmoid orbital fracture in a 32-year-old man that was managed without formal medial canthal tendon repair; instead, we used open reduction and internal fixation of the centr...

Infraglabellar transnasal bilobed flap in the reconstruction of medial canthal defects.

We investigated the effectiveness of reconstruction using an infraglabellar bilobed flap of transnasal skin and subcutaneous tissue for patients with medial canthal defects. Our noncomparative interventional case series involved 11 consecutive patients with medial canthal defects following Mohs micrographic excision of basal cell carcinoma. The medial canthal area was divided into 3 zones: zone...

Medial canthal ligament reattachment in skull base surgery and trauma.

INTRODUCTION The medial and lateral canthal ligaments represent fibrous condensations of tissue anchoring the tarsal plates to the medial and lateral orbital rims. The medial canthal ligament extends anteriorly to the anterior lacrimal crest, posteriorly to the posterior lacrimal crest, and superiorly along the frontal process of the maxilla and the frontal bone. The medial canthal ligament’s m...
